Clinically he appears well but complains of a pubic click & grind every time he tries to weight bear. A pelvic x-ray is done.
What does the XR show?
At first it looks odd..maybe a little rotated right? But on second glance, the pubic symphysis overlaps significantly and has a step. This is a case of overlapping dislocation of the pubic symphysis.
A rare disorder of of the pelvic ring, this injury occurs mostly from lateral compression. Any worse and the pubic body can wedge in the contralateral obturator foramen. When it gets that bad disruption of vessels and the ureter can occur.
It also goes without saying that this sort of injury signifies a moderate mechanism, so serial examinations and investigations of the abdomen are warranted.
